Why Women Lose Hair — and What Can Be Done
Hair loss in women is more common than most people realize — affecting approximately 40% of women by age 50, yet far less discussed than male hair loss. The causes are varied and often more complex than in men, involving hormonal fluctuations, autoimmune conditions, nutritional factors, and the physical trauma of certain hairstyles.
Female pattern hair loss (androgenetic alopecia) is the most common type, driven by genetic sensitivity to dihydrotestosterone (DHT). Unlike men, who typically lose hair in a defined pattern starting at the hairline or crown, women experience diffuse thinning across the top of the scalp — the hairline is usually preserved, but the part widens and the scalp becomes increasingly visible through thinning hair. This is classified using the Ludwig Scale (I–III).
Traction alopecia is caused by repeated mechanical tension on hair follicles from tight hairstyles — braids, weaves, extensions, tight ponytails, and buns. Over years, this tension damages and eventually destroys the follicles, causing permanent hair loss at the hairline and temples. It disproportionately affects women of African descent but occurs across all ethnicities. Once follicles are permanently damaged, the only way to restore the hairline is surgical transplantation.
Hormonal causes include post-partum shedding (telogen effluvium), menopause-related estrogen decline, thyroid disorders (both hypo- and hyperthyroidism), and polycystic ovary syndrome (PCOS). These conditions can cause significant temporary or permanent hair loss. When the underlying hormonal issue is addressed, some hair may regrow — but permanently damaged follicles will not recover without surgical intervention.
Alopecia areata is an autoimmune condition in which the immune system attacks hair follicles, causing patchy or diffuse hair loss. When the condition is stable and the patient has adequate donor hair, FUE transplantation can restore coverage — though patients with active alopecia areata are not candidates until the condition is controlled.
